- Your address must match what was entered during enrollment (e.g., "Avenue" instead of "Ave").
- Use your name as it was entered during enrollment. Note: last names are limited to first 15 characters.
- Check your Member ID. Member ID is 11 characters and starts with a zero. It is found on your ID card or Declaration page under "Policy (or Policyholder) Number."
Have an employer plan? Find your local Delta Dental based on the state where your employer is headquartered.
