Dental Plans
Annual maximum:$5,000
Deductible: $200 per person; (Does not apply to Diagnostic & Preventive services, such as cleanings and X-rays)
Comprehensive plan with a $5,000 dollar maximum that covers most major procedures; fillings are covered at 70% while preventive care is paid for in full.
Annual maximum: $2,000
Deductible: $100 per person; once per lifetime for as long as Policy is in force; (Does not apply to Diagnostic & Preventive services, such as cleanings and X-rays)
Get comprehensive coverage for a broad array of dental services (most dental procedures are covered). You get maximum savings along with a high dollar maximum. Preventive care is covered 100%, which means no out-of-pocket costs for exams, cleanings, X-rays and topical fluoride. This plan includes a lifetime deductible, and once paid, your plan will have no deductible.

This plan takes the guesswork out of dental expenses. You know before you go to the dentist what your share of the cost of any service will be. There are no deductibles, no waiting periods for benefits to begin, no percentages to worry about and no annual dollar maximums.

A plan that closely mirrors an employer-sponsored dental plan. The Enhanced Plan offers an ample benefit backed by the state's largest network and outstanding service you expect from the best dental carrier in Colorado.
Annual maximum:Year 1: $1,500; Year 2: $1,750; Year 3: $2,000 per person per plan year

An affordable plan that rewards members for maintaining dental coverage by reducing the patients cost sharing over a three year period - up to a $2,000 annual maximum.

Our most affordable plan focuses on prevention. In addition to services that cover preventive care like exams and cleanings, the Basic Plan also provides coverage for emergency services, fillings, and non-surgical extractions.

There are no maximums, no waiting periods, no annual deductible, and no claims to file. Payment is made directly from the patient to the provider.
