Locate your ID

  • Member ID: Found on your ID Card or Declaration page. It is a 10 digit number that begins with a zero. This can also be referred as your Subscriber ID. 

    For Northeast Delta Dental Members only:     If you purchased an Individual & Family plan through Delta Dental Covers Me, your Policy Number is listed on your Declaration Page. This number is the same as your Member ID shown on your ID card.

  • Reference Number: People who shop for benefits on the State of Washington Healthplanfinder™ website may find this number on your enrollment documentation or on your invoice.
