Dental Plans
Annual maximum:$5,000
Deductible: $250 per person; (Does not apply to Diagnostic & Preventive services, such as cleanings and X-rays)
Comprehensive plan with a $5,000 dollar maximum that covers most major procedures; basic procedures are covered at 75% while preventive care is paid for in full. Includes access to Delta Dental's largest PPO Plus Premier network.
Annual maximum: Year 1: $1,500; Year 2: $1,750; Year 3: $2,000 per person per plan year
Deductible: $50 per person; (Does not apply to Diagnostic & Preventive services, such as cleanings and X-rays)
An affordable plan that rewards members for maintaining dental coverage by reducing the patients cost sharing over a three year period - up to a $2,000 annual maximum.
Annual maximum:$2,500
Deductible: $100 per person; once per lifetime for as long as Policy is in force; (Does not apply to Diagnostic & Preventive services, such as cleanings and X-rays)
Comprehensive coverage and high dollar maximum plan with access to Delta Dental's largest PPO Plus Premier network includes implants, teeth whitening, veneers and occlusal/nightguard. Preventive care is covered in full with up to 3 cleanings per plan year. Once lifetime deductible is paid, there are no more deductibles on covered services.

Deductible:$50 per person up to $150 per family per plan year; (Does not apply to Diagnostic & Preventive services, such as cleanings and X-rays); Orthodontics have separate $100 lifetime deductible per person
Now available with orthodontic coverage, posterior composites and teeth whitening included. This plan closely mirrors an employer-sponsored dental plan with access to Delta Dental's largest PPO Plus Premier network.

Deductible: $50 per person up to $150 per family per plan year; (Does not apply to Diagnostic & Preventive services, such as cleanings and X-rays)
An affordable traditional design plan that covers most major procedures, posterior composites and teeth whitening. Basic procedures are covered at 60% and preventive care paid in full.

Deductible: None
Our most affordable PPO network plan that focuses on wellness. In addition to services that focus on wellness services like exams and cleanings, the Basic Plan also provides coverage for teeth whitening, emergency services, fillings and non-surgical extractions.

Eyeglass frames: $150 In Network Allowance ($80 Costco allowance)
Exams: $10 Copay for In Network coverage
Offers affordable vision exams and prescription glasses copays
Eyeglass frames: $200 In Network Allowance ($110 Costco allowance)
Exams: $0 Copay for In Network coverage
Offers the same great benefits and perks of the Essential Plan, plus more with a higher frame allowance
