Dental Plans
Annual maximum:$1750 up to $3500
Deductible: $50 individual/$150 family (Does not apply to Diagnostic & Preventive services, such as cleanings and X-rays)
Our most comprehensive plan with the highest annual dollar maximum. Includes orthodontic coverage for adults & children. Provides the highest level of coverage for basic restorative services.
Annual maximum:$1500 up to $3000
Deductible: $50 individual/$150 family (Does not apply to Diagnostic & Preventive services, such as cleanings and X-rays)
Similar to the Granite plan, but with a higher dollar maximum and increased coverage for basic services.
Annual maximum:$1250 up to $2500
Deductible: $50 individual/$150 family (Does not apply to Diagnostic & Preventive services, such as cleanings and X-rays)
Provides a balance of coverage for preventive, basic and major restorative services. Coverage for basic and major services set at the same 50% coinsurance level.
Annual maximum:$1000 up to $2000
Deductible: $50 individual/$150 family (Does not apply to Diagnostic & Preventive services, such as cleanings and X-rays)
Similar to the Acadia plan, but with a higher dollar maximum and increased coverage for basic restorative services.
Annual maximum:$750 up to $1500
Deductible: $50 individual/$150 family (Does not apply to Diagnostic & Preventive services, such as cleanings and X-rays)
A plan designed with a focus on prevention. Preventive care is covered at 100% by the plan. Also provides some coverage for basic and major services.
Vision Plans
Eyeglass frames: $200, then 20% off balance. Non-network reimbursement: $100.
