Dental Plans
Annual maximum:$750/$1,500/Unlimited (Year 1/2/3)
Deductible: $50 per person; (Does not apply to Diagnostic & Preventive services, such as cleanings and X-rays)
The Ultimate Plan features increasing coverage through the first three years of plan enrollment, with an unlimited annual maximum starting in year three. No waiting periods on day one, 100% coverage of preventive care, benefits for restorative and major services, and coverage for select cosmetic services.

Deductible: $50 per person; (Does not apply to Diagnostic & Preventive services, such as cleanings and X-rays)
The Elite Plan provides 100% coverage of preventive care and benefits for restorative and major services, plus $2,000 lifetime maximum for adult/child orthodontic coverage and a $2,000 annual maximum.

Deductible: $50 per person; (Does not apply to Diagnostic & Preventive services, such as cleanings and X-rays)
The Enhanced Plan provides 100% coverage for preventive care and offers cost sharing for restorative and major services like crowns, dentures, and bridges.

The Clear Plan removes the guesswork with fixed out-of-pocket costs, no waiting periods, no deductibles, and no dollar maximums.

The Basic Plan is the most affordable option covering preventive care, fillings, and non-surgical extractions.

This exchange-certified plan offers greater coverage for adults, plus highest coverage for services provided by a Delta Dental PPO dentist. This plan includes coverage for exams, cleanings, fillings, and major services. Benefits for children (up to age 19) are prescribed by the Affordable Care Act and may differ from adult benefits.

Vision Plans
Eyeglass frames: $250 allowance
Exams: $0
The Brilliance Plan offers a higher allowance for frames and lenses or contact lenses. Eye exams and popular lens enhancements like standard anti-reflective and scratch-resistant coatings are also covered in full.
Eyeglass frames: $150 allowance
Exams: $10
The Essential Plan offers an allowance for frames and lenses or contact lenses. Eye exams and lens enhancements will incur a co-pay. This plan helps keep monthly costs low, while providing coverage for essential vision care.
Network and providers
You can see any provider you like. However, you’ll save money if you see an in-network dentist. We have two networks: Delta Dental PPO (these dentists provide the largest discounts to save you the most money) and Delta Dental Premier (additional providers to choose from, but they offer smaller discounts). Together they make the Delta Dental PPO Plus Premier™ network with almost 151,000 providers nationwide to save you money.

Know your dental benefits
Regardless of the provider you see, you will be responsible for your plan’s deductible, coinsurance, and fees for services that are not covered benefits under your plan.
If you visit an out-of-network provider, you will be responsible for the difference between the provider’s charges and the amount your Delta Dental plan pays. If you enroll in the Clear Plan, there is no coverage if you visit a dentist who does not participate in the Delta Dental PPO Plus Premier™ network.
